Update Contact Information

To update or edit your contact information, please log into your licensee Gateway account, click on the top menu item called “My Profile,” and follow the prompts to update your contact information.

Name Change

To process a name change you will need to have one of the following available to upload:

  • A digital copy of your marriage certificate.
  • A digital copy of a divorce decree or court judgment granting the name change.
  • Alternately, you may also submit a digital copy of your current Driver’s License or Passport that reflects your changed name.

Log in to the licensee Gateway, and select “My Profile” in the top menu, and then choose “Name Change.” Enter and upload the required information.

Please note, you will have the ability to print your updated licensure ID card or certificate online, from the NCBDN licensee Gateway, after you receive email confirmation indicating that your name change request has been processed and approved.

Green notice iconNOTE: The NCBDN database is not “linked” with the Commission on Dietetics’ database, the Board for Certification of Nutrition Specialists’ database, or the American Clinical Board of Nutrition’s database. Thus, you must change your name with each entity separately.

ID Card and Receipts

Licensees may retrieve their ID card or receipts by logging into the Licensee Gateway and then clicking on the box representing an active license.

On the right side of the page, under “Options” is a button to “Print ID card.” Clicking on this will generate a PDF of an ID card that can either be saved to the desktop and/or printed.

Green notice iconNote: As of January 1, 2017, the Board no longer mails plastic ID cards to licensees.

In addition, above the “Options” area, there is a blue “View Receipts” button where all recent receipts are stored as PDFs that can be saved and/or printed.

Certificates: Print-Your-Own or Purchase Wall-Mount Quality

Print-Your-Own

You may log into the NCBDN Gateway at any time, click on your active license and on the following page, under your “Options” select “Print Certificate.” You can save it as a PDF or print it as you wish.

Wall-Mount

If you desire a wall-mount quality certificate, they are available for purchase for a duplicate certificate service fee of $30. This type of certificate is printed by our office on cream-colored cardstock with a gold embossed seal. You may purchase a wall-mount certificate by logging into the NCBDN Gateway; click on your active license and on the following page, under your “Options” select “Buy Certificate.” Submit payment online with your request.
