Hard Copy/Paper (FD-258 cards) Instructions for Obtaining a Criminal Background Check (CBC):
Step 1:
If you are not physically in NC, please contact your local law enforcement agency (police department or sheriff’s office) to determine the closest location that provides fingerprinting services; not all agencies do, so please call ahead. If you cannot locate a law enforcement agency to fingerprint you, you may use a private entity like Identogo or the UPS store. HOWEVER, please ensure you clearly indicate to the private fingerprinting business that you must leave your appointment with two paper FD-258 cards in your possession. Private entities often say “they” will send the fingerprints digitally; THIS WILL NOT WORK.
If your chosen fingerprinting location cannot provide FD-258 cards, you may print them on white copy paper or cardstock at home from this link. Or, you may request that cards be mailed to you from the NCBDN office at no cost, by emailing a request to: info@ncbdn.org.
Step 2:
Open, Print and review this document: Instructions & Authority to Release Information Form
Take these forms to your fingerprinting appointment/entity, as determined in Step 1. Fill out the top-portion of the two (2) FD-258 cards (mentioned in Step 1 above) using the instructions provided in the PDF form downloaded in Step 2. You must fill out all required fields, per the instructions. Once you have completed the fingerprinting, please mail the signed Authority for Release of Information form, & the two (2) completed FD-258 fingerprint cards to:

(Do not utilize special delivery methods such as “signature required,” to mail your Authority to Release Information form, fingerprint cards as this may actually slow down processing.)
